While there is an abundance of information of what is to be included in a qualified disclaimer and the rules concerned with making a qualified disclaimer, it is hard to find any examples or samples of actual qualified disclaimers.
Does anybody have a link to a site or sites that may have some sample qualified disclaimers? I'm only looking for the wording to use.

The disclaimer has to be "legal" in the state where it's executed. I've always had the attorney handle it and only glanced at it to make sure it had the elements in Sec 2518.
